| New Patients: I am not currently accepting new patients for treatment. I accept direct payment and do not bill insurance at this time. A receipt or "superbill" is provided which you may be able to submit for reimbursement from your insurance company. Your insurance company will consider me to be an "out of network" provider. The links below lead to files that my new patients need to review including important information about the office policies and fee schedules. Please bring a filled out new patient questionnaire and signed acknowledgment of receipt of privacy notice/ office policy to your first appointment. If you have trouble printing these out, please contact me. If you do not have adobe reader, which is necessary to view these files, please click here. If you continue to have difficulty, please contact me and I will be happy to provide the material to you.
